Cost or feesA financial screening is required to establish a monthly fee. Monthly fees range from $55.00 - $75.00 per month. In addition, there is a one time start-up fee of $100.

Eligibility descriptionFollowing the initial orthodontic screening, parents are required to participate in a financial screening of all household income: pay stubs, tax form 1040 from the most recent year, letters from employers, or verification of state benefits.
