The program offers 24/7 intervention and support plus links families and youth to existing community services.
Eligibility Requirements
Participant in Medicaid/Medi-Cal,
Foster youth (current or former),
Must be a ward of Alameda County Court with full-scope Alameda County Medi-Cal, at-risk for psychiatric hospitalization or have recently been hospitalized, experiencing crisis at home and/or at risk of losing home or foster placement.,
