Information Status
Phoenix House's Wraparound program provides support services for families with children who are experiencing behavioral problems or mental health disorders. Services consist of team-based intervention and developing an individualized plan based on respect for cultural values. Referrals are received directly from the child’s social worker or probation officer and are coordinated through the Department of Mental Health.

Must be a post-adoption youth, be on probation, or have an open case with DCFS,
