Unity Care's Transitional Housing Placement Program provides housing and supportive services for current and former foster youth ages 18-21, including probation youth, while they focus on their emotional, educational, and employment goals. All youth in this program will be educated with necessary life skills while living independently in Unity Care housing or a remote site of their choice.
