Information Status
Individuals can file a request with the Workers' Compensation Board when they want the Board to take a specific action in their workers' compensation claim or if they need to alert the Board to any problem or situation that is affecting their claim.
Eligibility Requirements
Request form should be used if you have a claim that is accepted by the insurer. It should not be used for claims that are controverted by the insurer or for claims that the insurer has not yet accepted.
