Rafiki Coalition offers an 11-bed transitional home for individuals who are diagnosed with HIV/AIDS, sober from substance use, and experiencing homelessness. The program includes case management and housing services for 6 months with a possible 6 month extension.

Eligibility Requirements
Experiencing homelessness,
People with HIV/AIDS,
Black or African American,
Must meet income requirements and be sober from substance use.
Documentation Needed to Enroll
HIV/AIDS diagnosis letter, current TB test or X-Rays, 2 letters of recommendation, proof of income, proof of COVID-19 vaccinations and boosters, copy of social security card, copy of CA ID, driver's license or passport, and mental health assessment.
