Hope Hospice offers patients and their families the compassionate care, insight, and guidance to help them navigate their way through the end of life with dignity. The Hope Hospice team of healthcare professionals, many with hospice and palliative care certification, provide a circle of support that addresses all of an individual’s needs: physical, mental, emotional, and spiritual.

Eligibility Requirements
Patients must be adults with a prognosis of six months or less, a prognosis that is certified by the patient’s primary care physician and hospice medical director, and both the patient and family desire palliative care as opposed to curative treatment.

Cost Details
Most health insurance plans including Medicare and Medi-Cal pay for hospice care. If no coverage is available, Hope Hospice will provide care through community donations.
