Calvary Hospice provides end-of-life care to patients with life-limiting illnesses whose care has transitioned to palliative and quality of life focused. The majority of care is provided in the home and short term inpatient care is provided in facilities.
Eligibility Requirements
Medicare will pay for hospice care if they have Medicare Part A; a physician certifies a terminal illness, with a life expectancy of 6 months or less; and Patient elects hospice benefit instead of standard Medicare benefits for the illness
