The Nurse Family Partnership and Field Public Health Nursing Program provide home visits to high risk prenatal and postpartum women and their newborns, chronically ill infants, children, and teenagers.
Eligibility Requirements
Women/girls,
Pregnant,
Mothers,
Resident of San Francisco,
Nurse Family Partnership Program: women who are pregnant with their 1st child, in their 1st or 2nd trimester, and meet income requirements. The Field Public Health Nursing Program: all pregnant or postpartum women living in San Francisco.
