Eligibility Requirements
People with HIV/AIDS,
Resident of Contra Costa County,
For case management, individuals must be eligible for Medi-Cal/Medi-Care or have some form of insurance in place. Also, individuals' total income must be below 500% of the Federal Poverty Level.
