Mental Health Case Management for Older Adults provides care and consultation to individuals 60 years of age and older who are living with a mental health condition. Services include assessment, counseling, placement, conservatorship monitoring, resource referrals, and more.

Service is intended for individuals with a chronic mental illness which interferes with daily living. Priority given to Latino and other un- or underserved populations and those at risk of out of home placement, institutionalization, or homelessness.,
