NMLEF provides financial assistance for vision-saving surgery to individuals who are unable to pay for the surgery and hospitalization. Financial assistance may also be provided for eyeglasses if they are included as part of the authorized surgery.

Must meet income limits and be unable to pay for surgery and hospitalization. Must have resided in the US for at least one year.

Application for Surgery and Hospitalization, Certificate of Surgical Providers, and Applicant's Permission for Surgery and Hospitalization and Certification of U.S. Residency forms.
