Eligibility Requirements
Participant in Medicaid/Medi-Cal, Must have LA Care, Anthem, or Kaiser and meet at least one of the following: mental health or substance use needs, complex medical needs, pregnancy or postpartum (up to 12 months), homelessness, foster care, or intellectual/developmental disabilities., Youth, Age: < 21 (or 26 if ever involved in foster care)
