MDHHS issues certified copies of birth and death certificates. MDHHS can also provide authenticated copies of birth and death records and birth or death verification.
Eligibility Requirements
Only the person/parent named on the record or legal guardian of the person named on the record may request a copy of a birth record that is less than 100 years old. Requests placed by a legal representative, power of attorney, or heir may also be granted.
Documentation Needed to Enroll
