Three to five year old children may be referred to the San Francisco Unified School District's Preschool Intake to determine their eligibility for special education services. Once an assessment is completed, an Initial Individualized Education Program is developed.

Proof of address, parent/caregiver questionnaire, verification of child's birth date, parent/guardian photo ID, written verification of hearing & vision screening results, copies of previous assessments, teacher input form if child attends preschool.

Cost Details
Qualifying families may pay on a sliding-scale subsidized tuition, based on family income. Families may qualify if parents/guardians are employed, attending school, medically incapacitated, participating in CalWorks, receiving child protective services.
