Individuals can apply to MDHHS by mail to correct or change a birth certificate. Individuals may request changes in name, sex, or parental registration.
Eligibility Requirements
May only be requested by person named on record (if over age 18), parent/legal guardian named on record, parent not named who can established that they should be added, legal representative, heir, or administrator responsible for filing original record.
