1) Be marginally-housed or homeless, 2) Be uninsured or receiving state health benefits (not from employment), 3) Have a source of income (SF-CAPP/General Assistance or SSI/SSDI) to pay program fees, 4) Receive referrals from Street Crisis, SFHOT, PES,
