Eligibility Requirements
Participant in Medicaid/Medi-Cal,
Available to all beneficiaries with full-scope Medi-Cal and to pregnant women. Transportation must be for services covered by Medi-Cal, and the beneficiary must have no other way to get to the appointment.
Documentation Needed to Enroll
Must know the appointment date and time.
