Get permanent supportive housing for individuals diagnosed with behavioral health disabilities

The Bridge Information Status

The Bridge provides permanent supportive housing for housing insecure adults and adults experiencing homelessness who are diagnosed with behavioral health disabilities. Case management staff check in with clients on an ongoing and as-needed basis.
Schedule
Individuals may remain in Bridge permanent supportive housing indefinitely. It may take several weeks or months to be placed in housing due to limited availability. Individuals must update and re-submit the HRA application every six months if they have not been placed in housing in the meantime.
Eligibility Requirements
Experiencing homelessness, 1) Must be a single adult; 2) Must be experiencing homelessness or be housing-insecure; 3) Must be diagnosed with a behavioral health condition; and 4) Must have established independent living skills and require minimal staff services., Adults (18+)
