Eligibility Requirements
Resident of NYC, Westchester, Putnam, Ulster, and Dutchess County,
Must be below 250% of the poverty line and a self-identified member of the LGBTQ-community
Documentation Needed to Enroll
Financial documentation of income and assets required for everyone in the household
