Receives SSI, Participant in Medicaid/Medi-Cal, People with disabilities, Resident of Napa County, Must be aged, blind, or have a permanent disability expected to last 12 months or more; be living at home; and have a licensed health care professional complete a Health Care Certification form indicating the applicant is at risk of out of home placement., Adults (18+), Seniors
