The Angels Hospice clinic offers hospice services for people with terminal illnesses. Hospice uses a team approach consisting of nursing, physician services, social services, hospice aides, physical therapy, and spiritual counseling for those with terminal illnesses.
Eligibility Requirements
Terminal, life-limiting diagnosis with a prognosis of 6 months or less. Approval of the hospice Medical Director and Attending Physician. Patient (or representative) consent to enroll in program. Able caregiver to provide for patient's daily needs.
