Paths to Health NM's YMCA Blood Pressure Self-Monitoring program is a 4-month program designed to help adults with hypertension lower and manage their blood pressure through home self-monitoring and individualized support and nutrition education.

Eligibility Requirements
Must be diagnosed with high blood pressure. Must not have experienced a recent cardiac event. Must not have atrial fibrillation or other arrhythmias. Must not be at risk for lymphedema.,

Cost Details
Program is valued at $250, $45 for participants who already have a digital blood pressure monitor. Digital blood pressure monitors are available for purchase for $30.
