Eligibility Requirements
Household income must be 200% below the federal poverty level. Patients may not hold any insurance or Medi-Cal, with the exception of Emergency Medi-Cal. Must have completed patient application prior to being seen (available at any clinic location).

Patients must provide proof of income or a letter certifying income.
