Information Status
Food for Thought provides free food and nutrition counseling for 3 months for Sonoma County residents who are released from an in-patient hospital or mental health hospital facility. Requires a referral from a doctor or medical provider.

Client hours are Tuesday–Friday 10:00 am to 4:30 pm.
Eligibility Requirements
Resident of Sonoma County,
Patients being discharged from an in-patient hospital or mental health hospital stay within two weeks of their discharge date, plus household members. Must score 6 or higher on DETERMINE Nutrition Checklist (screening is on back of the referral form).
Documentation Needed to Enroll
Referral checklist: welcome home nutrition for healing referral form, problem list, medication list, and recent progress note or medical hospital discharge summary.
