Eligibility requirements
Eligibility Requirements
Resident of Sonoma County, Patients being discharged from an in-patient hospital or mental health hospital stay within two weeks of their discharge date, plus household members. Must score 6 or higher on DETERMINE Nutrition Checklist (screening is on back of the referral form).
