Resident of San Mateo, Santa Clara County,
Qualify as income eligible (earn less than 50% of local area median income) or be enrolled with a qualifying program.,

Valid government ID, proof of residency, and proof of income required for application of assistance. Itemized estimate of veterinary care required for final approval, not required if you don't have it yet.
