Apply through mail by printing and completing the Medi-Cal application (offered in various languages), which can be found on HHS' website. Mail completed forms to PO Box 4160, San Rafael, CA 94913. Completed forms can also be faxed to (415) 473-3554.

Eligibility Requirements

Families with children, seniors, persons with disabilities, pregnant, children in foster care, former foster youth up to age 26, and individuals with diseases (such as tuberculosis, breast and/or cervical cancer, or HIV) may be eligible for Medi-Cal.

US citizen or permanent resident (Green Card Holder)
